Your photograph and identifying information will be distributed to all New Jersey casino
licensees, sportsbooks and internet gaming platform providers The information contained on
your Request Form, the Atlantic City Casino Gambling Self-Exclusion List and the Internet
Gaming Self-Exclusion List maintained by the DGE is not open to public inspection and every
effort will be made to maintain its confidentiality. However, the DGE, the casinos, racetrack
sports wagering facilities and the Internet gaming license permit holders are not liable for any
disclosures of any information, other than a willfully unlawful disclosure. Certain limited
disclosures by the casinos and Internet gaming license permit holders are permitted. For
example, the casinos and Internet gaming license permit holders may inform certain of their
agents, including cash advance services and junket representatives that you are on the
Voluntary Self-Exclusion List and the Voluntary Internet Gaming Self-Exclusion List for the
purposes of denying you gaming-related services. Also, the casinos and Internet gaming
platform providers are permitted, by law, to disclose to affiliate gaming entities in this state or
other jurisdictions that you are on the Voluntary Self-Exclusion List for the limited purpose of the
proper administration of responsible gaming programs administered by the affiliated gaming
entities.

You should be aware that casino operators, sports wagering operators and internet platform
providers might enact responsible gaming programs that are stricter than New Jersey’s
Voluntary Self-Exclusion program for both land-based and Internet gaming. By way of example
certain Atlantic City casinos may enact a responsible gaming program in which persons signing
up for state self-exclusion lists are banned from the casino’s properties worldwide for the length
of their state self-exclusion term. Such ban could include its gaming, hotel and entertainment
venues in other jurisdictions The terms and existence of any such responsible gaming program
could change, and the DGE is not responsible for keeping you informed of such changes.

In accordance with Section 7 of the Privacy Act, 5 U.S.C. 552a, disclosure of your social
security number to the DGEis voluntary. Failure to disclose your social security number is not
grounds for denial of your request for self-exclusion. However, a delay may occur in notification
to Internet gaming platform providers for a person who has not supplied a social security
number. A failure to supply your social security number may compromise the effectiveness of
self-exclusion procedures implemented by Internet gaming platform providers. The request for
your social security number is made pursuant to the Casino Control Act, N.J.S.A. 5:12-1, et seq.
Your social security number will be disclosed to the Internet gaming license platform providers
for their use in identifying you as a self-excluded person to prevent you from gambling, to deny
you credit, check cashing and similar privileges, and for forfeiture purposes. Submission of your
Social Security Number is required if you are requesting to be self-excluded from Internet
(online) gaming through online registration. If you do not want to disclose your social security
number, please contact the Division of Gaming Enforcement at 1-833-788-4DGE to schedule an
in-person or video conference.

When registering online, you must choose one of the following two options in connection with
the length of time that your name must remain on the Voluntary Self-Exclusion List:
one year or five years. You cannot request the removal of your name from the Voluntary Self-Exclusion List
until the expiration of the one-year or five-year period from the date you submitted your Request
Form. Additionally, your name will continue to remain on the Voluntary Self-Exclusion List until
you request its removal. If you wish to choose a lifetime exclusion option, you must contact the
Division of Gaming Enforcement at 1-833-788-4DGE to schedule a video or in-person
conference.

It is your responsibility to refrain from gaming activities.
DGE, casinos, racetrack sports wagering operators, New Jersey Racing Commission and Internet gaming platform providers
are not liable for any acts or omissions in processing or enforcing your request for self-exclusion,
including the failure to withhold your gaming privileges. However, if you are found
gambling at a casino, racetrack sports wagering facility or on an Internet gaming site, you will be
subject to forfeiture of any winnings, including any chips or tokens in your possession, electronic
credits on your account, funds on your on-line gaming account which were the result of your
gaming activity and you will be escorted from the gaming area or removed from the Internet
gaming site. The DGE, the casinos, racetrack sports wagering operators and internet gaming
platform providers also are not liable for any acts or omissions in processing or enforcing any
later request by you to be removed from the Atlantic City Casino Facilities and Internet Gaming
Self-Exclusion List (s).

Act at N.J.A.C. 5:12-71.2 and 71.3, as well as the regulations set forth at N.J.A.C. 13:69G-2.1
through 2.5. Copies of these rules are available upon request from DGE personnel and are also
available on the DGE's website. Rules may be changed without prior notice to Voluntary Self-
Exclusion program participants.
