What is SBI Number? Is it a required field
A State Bureau of Identification number (SBI#) is a unique number assigned by the NJ State Bureau of Identification for use in identifying all criminal and non-criminal finger prints kept on file within the bureau. Not all applicants will have an SBI#, and unless you are applying for firearm purposes it is not necessary to provide one.

What is an ORI Number and how can I find the ORI Number?
The ORI (Originating Agency Identifier) Number is a unique number assigned to each Police Department and State Agency. While filling out the online form you need to enter the ORI Number for the licensing authority that is responsible for the type of licensing you need, for your area of residence. You must first obtain the correct ORI Number from the licensing authority. An incorrect ORI Number may result in the non-processing of your form.

Are all local licensing authorities accepting Form 212A online?
Not all local licensing authorities may be set up to accept and process Form 212A online.

How can I check whether a particular licensing authority is accepting Form 212A online?
The online Form 212A starts with the ORI Number. If the concerned licensing authority is not accepting online requests, you will not be able to proceed with the Form 212A submission. You need to make sure that you are applying with the correct ORI number after consultation with the concerned local licensing authority. The name of the local licensing authority is displayed when a valid ORI Number is entered.

What if I select a wrong ORI while filling out the online Form 212A?
The name of the local licensing authority is displayed when a valid ORI Number is entered. If you submit Form 212A for an incorrect licensing authority, it will not be processed. You will need to reapply and pay again. It is therefore, very important to first consult the concerned local licensing authority and get their ORI Number before submitting the online request.

You will not receive the Criminal History Record Information requested. This information is used exclusively by the local licensing authority to complete your request. However, upon successful payment for the transaction you will be presented with a confirmation page as well as receive the confirmation email for the request submitted.
