Consultant Prequalification Application Instructions (form 48A)
The Professional Services Prequalification Application 48A will be thoroughly evaluated by DPMC’s Prequalification Unit.
Prequalification disciplines and ratings are determined based upon the following factors:
1. Firm experience, staff, and capabilities. All applicant firms must be in business for at least two years.
2. Financial stability and viability as demonstrated by review of financial statements
3. Required professional licenses/certifications, Board Authorization (if applicable), and other NJ documents as required
DPMC prequalifies a single office , not an entire company. Prequalification will be based entirely upon the capabilities of the office
indicated in Box 1. Staff from a single satellite office in proximity to the prequalified office may be applied as well, only for the
purposes of increasing your ratings. Applicant firms with multiple offices are encouraged to apply for prequalification for the office
which has the most capabilities (NJ licensed staff, etc.) for the disciplines they are applying for.
A firm’s prequalification rating may vary by discipline. Prequalification dollar ratings range from $100,000 to $25,000,000. DPMC
also issues an “UNLIMITED” rating, and an “NA” rating, for disciplines in which a dollar threshold does not apply.
Following prequalification, the firm will be eligible to apply for DPMC professional consultant services contracts, or serve as a sub-
consultant on such contracts, within its approved disciplines, with a construction cost estimate equal to or below the firm’s
prequalification rating.
A firm’s prequalification rating will be valid for a period of two years. The expiration date will be noted on the “NOTICE OF CONSULTANT PREQUALIFICATION,” Form 48AN.

Materials Testing Laboratory Application 48T
The “Materials Testing Laboratory Application” (48T) will be thoroughly evaluated by DPMC’s Prequalification unit, and the lab’s
materials testing prequalification based upon the following factors:
1. The laboratory’s experience, staff, and capabilities. All applicant labs must be in business for at least two years.
2. Financial stability and viability as demonstrated by review of financial statements.
3. Required laboratory accreditations/memberships, staff certifications, materials testing equipment, etc.
4. All other applicable State documentation as required.
The laboratory will be eligible to apply for professional consultant service contracts for material testing or to serve as the sub-
consultant for materials testing for a professional consultant services contract within its approved disciplines.
The laboratory’s prequalification will be valid for a period of two years. The expiration will be noted on the Notice of
Prequalification form 48TN.
